1Volunteer Fundraising Board Member

Reports To: Social Media/Marketing Manager

Volunteer position


Exponents is seeking to fill five (5) Volunteer Fundraising Board Member positions reporting to the Social Media/Marketing Manager.

The Fundraising Board will plan and sets fundraising goals and objectives; develops policies and procedures; and guides the implementation of a range of fundraising initiatives (i.e. individual giving, special events, annual appeal) aimed at diversifying Exponents revenues for the purpose of continued service delivery. Members are expected to contribute their expertise and contacts, attend and facilitate meetings, brainstorm to come up with sound ideas to be presented to the management team for approval, and implement fundraising endeavors from start to finish.

2Sr. Treatment Counselor

Reports To: Clinical Director

FLSA Status: Exempt

Salary Range: $46k - $50k (commensurate w/experience)

Position: Sr. Treatment Counselor (CASAC-2 or higher)

As a member of the clinical team provides patient care, upholds professional clinical standards, and provides expertise in the treatment of chemically dependent people from a diversity of populations.

Position Activities:


• Conducts comprehensive intake assessments including LOCADTR

• Refers clients to alternate services when appropriate

• Conducts vocational assessments

Case Management:

• Responsible for the clinical case management of assigned caseload as primary counselor

• Prepares treatment plan and updates

• Monitors client’s progress in program and reports progress in clinical meetings

• Monitors clients social service needs and arranges for services

• Monitors & address issues of toxicology reports with client

• Liaisons with outside agencies such as parole, probation, HRA, child protection as needed by individual case assigned

Individual Counseling:

• Conducts individual counseling sessions focusing on chemical dependency, social adjustment and life stabilization issues

• Conducts individual crisis intervention sessions

Group Counseling:

• Responsible for communication to clinical team for group track assigned as primary group facilitator

• Conducts chemical dependency education groups and process groups/

Client Recruitment

• Establish, coordinate and facilitate bi-directional referral agreements between Exponents’ Outpatient Drug Treatment program and external service providers.

• Schedule and conduct informational presentations to external service providers for the purpose of networking and client recruitment.

• Responsible for ensuring client census goals are achieved and maintained though program promotion and client outreach.

Clinical Team Participation:

• Attends clinical case conferences

• Assists Clinical Director in program development

• Reports to Clinical Director

Qualifications: CASAC-2 or MSW; a minimum of 3-5 years experience in the substance abuse field; good written and verbal communication skills necessary: Knowledge of Medicaid billing and Managed Care plans a plus; awareness of Mental Health issues; ability to promote Agency to providers and recruit clients; computer skills required.

3Care Manager

Reports To: Care Management Coordinator

FT-Temp – Non Exempt

Salary Range: Commensurate w/ experience


The Care Manager will provide direct care coordination services to Health Home participants that reside in the New York City area; these individuals may have a co-occurring substance abuse diagnosis, HIV/AIDS and other chronic illnesses. Care Manager will assist consumers in increasing their capacity to live as independently as possible and remain an active part of the local community.


•Provide care management/advocacy services to assigned caseload.

• Provide coaching to consumers.

• Participate as a member of a multiple disciplinary treatment team to help facilitate the recovery process of the consumers by providing individualized continuity of care

•Conduct assessments and develop care plans with consumers.

•Escort consumers to scheduled/walk-in appointments and program services.

•Maintain accurate and updated case records documenting consumer's progress in treatment and response to treatment services in accordance with the programs record keeping policy and procedures in Crimson Care Management tracking system.

•Develop consumer's care plan in conjunction with the Program Director, client, family, referral sources and other community-based agencies to which the consumer may be referred for continuing care.

•Participate in outside meetings as assigned.

•Attend and participate in regular staff meetings/In-Service trainings, case conferences and individual supervisory meetings.

•Provide regular home visits to assist consumers with building ADL skills in preparation of obtaining or maintaining independence.

•Input appropriate information into documentation and reporting platforms (e.g. Crimson Care Management and the Medicaid Analytics Performance Portal)

•Submit timely Medicaid billing as applicable to program.

•Maintain a caseload of assigned consumers.

•Perform other relevant duties as assigned.


• Experience and knowledge of the homeless, mentally ill, MICA and HIV/AIDS population preferred. Knowledge of entitlements and HRA 2010E a plus. Must have continuing ability to work as a member of the team providing housing and services to a special needs population.

• Strong written and oral communication skills. At least two years experience in Case Management preferred. Proficiency in Microsoft Office computer programs.

• Willingness to work with individuals who are Persistently Mentally Ill, Homeless and Chronically Ill.


• Bachelors Degree in Human Services or related field required.

• Bi-lingual (English/Spanish) ability and/or experience in the Mental Health field is a plus.
